All requests for vital records must be made to the Office of Vital Records at the Oklahoma Department of Health. To be eligible to apply for a vital record, a person must be a subject of that record or must be either the next-of-kin or legal representative of the subject. The Office of Vital Records charges $5 to process a birth certificate and $10 to process a death certificate.
